Please complete the form below and click Submit once completed.
Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter, please call reception to leave a comment, or make an appointment to see the appropriate person.
When we are closed

Online Consultations
You can still submit routine queries via our Online Consultation service - you can read more about this on our Appointments page.
NHS 111.
111 is the NHS non-emergency number. It’s fast, easy and free. Call 111 when you need medical help fast but it’s not a 999 emergency.
You will speak to a highly trained adviser, supported by healthcare professionals. They will ask you a series of questions to assess your symptoms and immediately direct you to the best medical care for you.
NHS 111 is available 24 hours a day, 365 days a year. Calls are free from landlines and mobile phones.

Accessibility
The Surgery car park has four parking spaces suitable for disabled patients.
The entrance to the Surgery has an automatic door and a wheelchair is available at the entrance. An induction loop also available from reception. We have accessible toilet with changing facilities.
For patients with hearing difficulties we have a system of adding alerts to patients notes on the clinical system so GP and staff are aware of patients needs.
Enhanced Access appointments
For evenings and weekends.
GP practices are working together to provide Enhanced Access appointments to our patients. This means that your extended access appointment may be with a different GP, nurse or based in a different location or practice than you are used to, and some of these appointments are telephone only.
These appointments are pre-bookable by contacting the surgery during our opening times – when calling please ask for an Enhanced Access appointment and our receptionists will help you to find a convenient appointment.
